Early Start Coordination provides Christ-centered, compassionate service coordination for children (birth to three) in New York’s Early Intervention Program. Our mission is to support families and ensure every child receives the services they need, embracing all races, colors, origins, and religions equally.
Our vision is to create a nurturing, inclusive community where every child and family experiences the love of Christ. We specialize in comprehensive early intervention services that support young children’s growth and development. Our dedicated team works closely with families to create personalized plans that address each child’s unique needs, ensuring they receive the best start possible.
Comprehensive Service Coordination
We offer continuous and professional coordination of early intervention services to ensure your child receives the support they need during their early developmental years. Our coordinators monitor service delivery, facilitate communication between families and providers, and arrange necessary evaluations and assessments.
Personalized IFSP Development and Review
Our team collaborates closely with you to develop meaningful and achievable goals in your child’s Individualized Family Service Plan (IFSP). We regularly review and update the IFSP to reflect your family’s priorities and adapt to your child’s evolving needs.
Transition Planning
We specialize in guiding families through the Committee on Preschool Special Education (CPSE) transition process. Our coordinators work diligently to develop and implement individualized transition plans, ensuring that your child smoothly moves from early intervention programs to preschool special education services. We support your family every step of the way, helping you understand procedures, obtain necessary consents, and prepare new service providers for a successful handover.
